I believe there are some more 'grey' strategies you could try - like running ads to a separate blog about the service that links to your main site. Caveat: I haven't tried this but I've seen it mentioned!

You cannot run addiction services outside of the US period.  
The Google AdWords Group is where you can always find questions, answers, advice, reviews & recommendations from other community members about successful search engine marketing (SEM) ads through Google AdWords.
