I think like most it depends on competition and where you're at. I'm in Iowa and my wife is a therapist. I set up some simple ads for her and for under . 50 a click she was getting phone calls and leads. I'm sure that would be much much higher in more competitive markets.

Anecdotally, people tend to prefer forms and messaging services over calling. But a lot of it really depends on modality focuses, specialties, location/competition, price, insurance, etc. The broadly applicable trick is to minimize the friction between the user searching and getting in touch with the therapist. 2 step forms work well for that since you can capture some basic info, and then ask for more without missing the whole lead if they decide the second step is too much. But again, making it just easy peasy to talk to someone is the key.
