How do you determine if a keyword would be regarded as close match (IE there's no point really making a separate ad group) or not? E. g. Lesvos and Lesbos. Will Google see these as the same? Should I make separate ad groups. In the old day, sure, now?

Personally, I wouldn't bother with those minor details. Unless there's volume of those clicks. Both you and the client knows its the same meaning but just bad or different spelling. I'm grateful that Google is allowing those matches otherwise we wouldn't be in front of customers.

There's volume on both. How I do I know for sure if Google sees them as the same?

@coble You decide that. Look at the conversion values, does it gen leads or sales. If not then you can always make it a negative?
