Looking for some insight on why keywords with my business name are getting marked with low quality score? I only have this issue on Call Only ads.

It is normal, because you cant enter to website with call only ads, you will use call only ads with normal search campaign, add one in to your ad group, from advanced bid adjustment increase call bids %5
