Happens when you change campaign or ad set settings which makes your ad incompatible with the campaign or ad set. Also happens when pasting an incompatible ad taken from another campaign. If you wrote a lot of stuff on the ad its gone forever. This is an FB bug. It shouldn't destroy your edits without warning.
