Apologies if this has already been asked, but cannot see it in the posts I've looked at. I bought my Doorbell 2 from Costco and have it installed and working. However the deal came with 6 months Ring Protect, but there are no codes on / in the box. Having set it all up the app is now telling me it expires in less than a month. How do I activate / get the 6 months?

Call Costco and report the issue to them

Call ring support. They know what your serial number is, when you set it up and how much plan is owing to you.
